Who’s Eligible
Adults, 18 years of age or older who have mental illness, and as a result of a mental illness, have challenges that interfere with at least one of the following: education, work, self maintenance, understanding his or her illness, money management, social/family relationships, are candidates for ICM Services.
Individuals receiving ICM Services usually are people with the following diagnoses or problems:
- significant disability from other mental illnesses that are not helped by traditional outpatient models
- bad experiences in the traditional system
- co-occurring substance abuse problems, self-medication histories
- difficulty getting to appointments on their own
- limited understanding of the need for help
Making a Referral
Referring agents may include, but are not limited to, medical staff, social workers and/or case managements from inpatient or other cris units and mobile crisis teams. All referrals for ICM should be sent directly to Elwyn for review and assessment. After the assessment, elwyn will determine if an applicant meets the admission criteria. The Elwyn staff is avilable to assist in the referral process.
Funding
ICM accepts funding from Magellan Behavioral Health and Lehigh County and Northampton County. Other types of funding arrangements, including those from behavioral health managed care insurers, private health insurance companies, and HMOs, as well as funding from County human services departments and private pay, may be arranged.
