The Extended Assessment Program offers up to 24 hours of assessment and case management to children in Delaware County. The program links children with the services that will best aid their overall growth and development.

A fully-credentialed Master’s-level clinician and a clinical case manager oversee each child’s case and collaborate with the school, community, and home to determine the most suitable resources for the child. Recommendations may include continued assessment, EPSDT/Wraparound evaluation, or another level of care.
