PROGRAM OVERVIEW
This program is designed to assist the families of Delaware County children and adolescents receiving mental health services in the referral process for residential services. The Residential Service Manager will work with the families form the initiation of the referral process through the successful reunification with the family following residential services. Delaware County children and adolescents currently not in a out-of-home placement outside of the county, will remain the responsibility of the RSM, Delaware County, HealthChoices and the other treatment providers.
SERVICES
- Coordinate referrals to residential services
- Creation of a crisis response plan for the family and providers
- Ensure family visitation including appropriate follow-up.
- Provide monthly summaries of services to family
- Coordinate referral with community providers prior to discharge.
- Ensure services and in-place prior to residential discharge.
- Assess and address issues which create trauma for children in the placement process
- Ensure all services provided are family driven planning and youth guided
- Encourage family members in the treatment process
ELIGIBILITY
Children and adolescents must be residents of Delaware County and currently receiving mental health services managed through DelCare. Children and adolescents served by RSM must be under 21 years of age, possess a diagnosis of mental illness, Autism Spectrum Disorder or mental retardation, where their conditions interfere with their daily functioning and be at-risk for our-of home placement.
MAKING A REFERRAL
Referring agents may include, but are not limited to, medical staff, social workers and/or community case managers of children at risk for out-of home placement.
