Who's Eligible
Adults, 18 years of age or older who have mental illness, and as a result of a mental illness have challenges that interfere with at least one of the following: education, work, self maintenance, understanding his or her illness, money management, and social/familial relationships, are candidates for ACT services.
Individuals needing ACT services usually are people with the following diagnoses or problems:
- a mental illness that is not helped by traditional outpatient models
- bad experiences in the traditional system
- co-occurring substance abuse problems, self-medication histories
- difficulty getting to appointments on their own
- limited understanding of the need for help.
Making a Referral
Referring agents may include, but are not limited to, medical staff, social workers and/or case managers from inpatient or other crisis units and mobile crisis teams. All referrals for ACT assessment must be approved by the respective County office or Magellan Behavioral Health. After the assessment, the ACT clinical team will determine if an applicant meets admission criteria and contact the funding source for approval. ACT staff is available to assist in the referral process.
Funding
ACT accepts funding from Magellan Behavioral Health and Lehigh County and Northampton County. Other types of funding arrangements, including those from behavioral health managed care insurers, private health insurance companies, and HMOs, as well as funding from County human services departments and private pay, may be arranged.
