Additional support may include learning to use public transportation, accessing community resources, scheduling and attending appointments, participating in social and leisure activities, and developing financial management skills. Staff assist with medication administration and provide instruction and support to help residents safely manage and adhere to prescribed medications. The program also helps coordinate healthcare appointments, including psychiatric, behavioral health, medical, and dental services. Residents may continue working with their current physicians and providers, or staff can assist with identifying and selecting healthcare professionals based on individual preferences and needs. Support extends beyond appointments and may include monitoring health concerns, individualized dietary accommodations, and wellness planning. Residents also have access to highly personalized care coordination designed to support overall health, stability, and long-term well-being.
