Assertive Community Treatment (ACT)

Elwyn’s ACT program provides intensive, community-based services designed to support recovery, reduce institutional care, and strengthen community connections.

Community Support Team (CST)
Elwyn’s Community Support Team (CST) delivers intensive, community-based support for individuals with complex mental health and substance use needs.
Features:
- Serves up to 48 individuals
- Integrated mental health and substance use services
- Team-based, in-community interventions
- Focus on recovery, stability, and community integration
Drop-In Center
Elwyn’s Drop-In Center offers a welcoming space for individuals to build connections, develop skills, and support their recovery journey.
Features:
- Clubhouse and peer-run recovery model
- Recovery counseling and peer support
- Creative arts (Studio 35) and recreational programming
Psychosocial Rehabilitation Program (PSR)
Elwyn’s PSR program is a center-based service that helps individuals build skills needed for greater independence in daily life.
Features:
- Psychoeducational and skill-building activities
- Strengths-based, person-centered approach
- Focus on coping skills, stability, and independence
Tailored Care Management (TCM)
Tailored Care Management (TCM) provides coordinated, whole-person care for eligible Medicaid members. A dedicated care manager works with a multidisciplinary team to support physical health, behavioral health, pharmacy needs, long-term services and supports (LTSS), and access to community resources—helping individuals achieve better health outcomes and greater well-being.
Community-Based Capacity Restoration Program (CBCRP)
Elwyn’s CBCRP provides community-based treatment for individuals unable to proceed to trial due to behavioral health needs, focusing on restoring capacity and improving stability.
Features:
- Individual and group counseling
- Psychiatric evaluation and medication management
- Court coordination and discharge planning
- Goal of restoration within approximately 90 days
Forensic Services
Forensic Services provides clinical and case management support for individuals involved with the justice system. Services include: mental health and substance use counseling, crisis intervention, psychiatric triage, care coordination, and connections to essential resources such as healthcare, housing, employment, food assistance, and recovery supports. The program also helps individuals successfully transition from incarceration back into the community.
