In Home Supports

All people, despite their challenges, deserve opportunities to build productive and meaningful lives. The In Home Supports program strives to provide services for people with challenges in their own homes and communities. The program addresses and meets the consumer choice and demand for services that help individuals develop their lives on their own terms, improving quality of life and utilizing the concept of self-determination. The type, frequency, and times of service are tailored to the needs of the individual. In Home Supports provides staff to an individual in his or her own home, or in the home of a family, friend, or a relative to allow for an individual to stay with natural supports as long as possible and/or to age in place.

In Home Supports Provides:

  • Community Integration
  • Socialization Skills Training
  • Respite

General Admission Criteria:

  • Children and adults
  • Living independently or with family
  • Diagnosis of disability (ID)
  • Funding (through OID or privately)
